The Toward Home Service Model

The Toward Home Alliance is committed to working with its Alliance partners: Baptist Care SA, Lutheran Care, Mission Australia, Sonder and The Salvation Army; the South Australian Housing Authority; and other organisations in the community and private sectors, to tackle the issue of homelessness.

Toward Home offers services to people experiencing or at risk of homelessness in the Adelaide CBD, inner and outer southern metro and Adelaide Hills.

Guided by our Mission to prevent and end homelessness in South Australia, we created the following specialised services for clients.

Toward Home’s services are:

Access

This is our ‘front door’, providing initial intake, assessment, and referral. This service is available Monday-Friday, 9am-5pm, by contacting 1800 809 273 (free call from all phones).

Prevent

Prevent focusses on keeping and supporting people safely in their current accommodation.

Divert

Divert focusses on rapid rehousing to reduce the length and severity of homelessness.

Resolve

This program aims to resolve the experience of long-term homelessness through assertive outreach, health and wellbeing, and the provision of long-term housing with accompanying support.

Toward Home Wardli-ana

A culturally appropriate service dedicated to Aboriginal and Torres Strait Islander peoples and communities.

Wellbeing

This program supports people in addressing health and mental health issues that may be impacting on maintaining or securing a home.

Specialised health and mental health assistance

Working with clients alongside the teams delivering Prevent, Divert, Resolve and Wardli-ana, is the team of health professionals from Alliance partner Sonder. They provide assistance to clients to address any health and/or mental health challenges which may be a barrier to them securing and maintaining a safe and sustainable home. The team includes Peer Workers with lived experience of homelessness and/or mental health challenges.

The Toward Home Access team

When a client contacts Toward Home, they are welcomed by our dedicated and professional Access team. The team is contactable via the Toward Home Access line, 1800 809 273 (Monday-Friday, 9am-5pm). Our Access team listens to each client's story and supports them to connect with the Toward Home service that best suits their specific needs. Clients can also connect with Toward Home services and supports by visiting Access staff based at WestCare Centre, Southern Pathways Centre, and Hutt St Centre. Through our no wrong door approach, we aim to ensure clients only need to tell their story once, regardless of how they connect with us.
